Dana Medicare Claim Procedures

The Dana Medicare plan basically offers broadly two modes of operation, Direct Billing and Reimbursement explained in detail below :

Reimbursement :-

Medical Claims for treatment sought in medical clinic, medical centers and hospitals that are not a part of our Warba network.

1. The claim form should dully filled, attested by the attending doctor and submitted to Warba along with the supporting document listed  below to Warba Takaful Claims Department. :-

	The medical reports in case of stay at the hospital .
	Medical prescription .
	Treatment receipt .
	If the doctor required any medical investigations , the form should be enclose with lab results along with charges paid .

2. The Failure to fill and stamp the claim form by the medical attendant shall lead to rejection of the claim .
3. The claim form shall be sent along with the required documents to the flowing address :

Warba Takaful Insurance Co. shall initiate required procedures to settle the claim within 15 days from the receipt date of receiving the form with all necessary documentation .

Direct Billing

Direct billing is applicable only when treatment is sought at  hospital , clinics , and medical centers are mentioned in  W M I C network table above.

	The patient  should produce the Dana Medical Card to receptionist .
	Present your Identity Proof ( Civil ID , License etc...)
	The receptionist shall verify your ID and hand over a claim from to be filled by the doctor .
	The patient should fill the claim form ( Patient Details Section ) .before handling over to the doctor.

 The patient  shall not pay any additional charge , provided the treatment/services available are covered under the insurance plan .

Prior to the Inpatient treatment, authorization/approval should be obtained from W M I C, failing which the claim shall not be considered. The treating hospital should arrange to send the preauthorization form to Warba Takaful Insurance Co, ( Private Medical Dept. ), to initiate the necessary procedures .

When approval is obtained from the W M I C , the hospital shall notify the patient thereof

In both the Direct Billing & Reimbursement modes of Claim, Inpatient and Outpatient Treatments are covered.
