	List of all employees to be covered “in Arabic” providing  details of  (Name, Sex, Nationality, Residence Expiry Date, Civil ID Number).
	Bank Guarantee

General Conditions

The insured hereby undertakes to observe the conditions and provisions of this policy and any attached endorsements; however, the insured will not have the right to go beyond these conditions and provisions unless a written consent is obtained.  The company will sustain treatment costs of the insured (s) during the validity period of the policy; however, the obligation of the company towards any insured (s) will cease as from the expiry date of the insurance even if the sickness or the accident has been treated before the expiry date of the policy.  The company hereby observes the protocol signed between the Ministry of Health and National Insurance Companies dated 02/07/2000 and the attached appendix as well as the text of article 4 Law No. 1 for 1999 regarding health insurance whence health services covered by the policy and those uncovered services.  The insured undertakes to pay the fees of the medical insurance card. The company is not responsible for any mistakes resulting from occupational practice or the negligence of any Doctor, Nurse, Hospital, Pharmacy, or any other Medical institution.  The company reserves the right to increase the insurance premium upon annual renewal of the policy. Further it also reserves the right not to renew the policy for another insurance period.  The company undertakes to pay treatment invoices issued by the Kuwaiti Ministry of Health only; however, it will not pay any invoices issued by any other authority, whether it is Government or non-Government.  This policy doesn't cover KD 1/- (Kuwaiti Dinar One) charge payable at out patient and KD 2/- (Kuwaiti Dinar Two) Hospital admission fee or any amendments that may occur to these fees.  The company will not cover any person who is not mentioned in the insurance policy, even though he is a family member of the insured or employed by the insured.  The policy will be considered as cancelled and the premiums will be non refundable in case any false statements or financial facts are not mentioned or stated clearly.  This policy includes the insured and those added to his/her passport under "family residence" if they are mentioned in this policy at the date of contracting. In case they are not mentioned in the policy, the insured has the right to add any of them during the insurance year as from the date that will be specified by the insured and against full annual premiums. The cancellation of any insured (s) from the policy will not result in refunding any insurance premium. The approved currency of this policy is the Kuwaiti Dinar. The male terms include the female terms for the same meaning.  The company will have the right to inquire about the insured (s) at any time from the Public Authority for Civil Information.  The courts of the State of Kuwait have the exclusive jurisdiction over the disputes that may arise from the interpretation or implementation of this policy.

Cases covered by the Policy

The policy covers the Health Services provided for under article 4 of Health Insurance Law No. 1 for 1999 as follows:  Medical examination and necessary treatment in the clinics by general practitioners and specialists. Laboratory tests and X-rays.  Surgeries except plastic Surgery.  Treatment, medication costs and the expenses of hospital admission at ordinary and emergency cases.  Ordinary teeth treatment. Medicines.  On the other hand this cover is subject to the procedures and rates specified for medical treatment costs provided for the insured (s) in the hospitals and clinics according to the protocol signed between the Ministry of Health and National Insurance Companies on 02/07/2000.

Cases that are not covered by the Policy:

Psychological diseases. Organs plantation, inclusive teeth plantation. Plastic surgery except skin plantation in case of burns. Patients suffering from permanent diseases and those who need continuous treatment for long period.  Physiotherapy in out patient. Addictions treatment.  I.V.F – ET (in Vitro Fertilization – Embryo Transfer) Rental failure and hepatitis. In this case the patient will be treated till the expiry date of the insurance should failure occurs during the validity period of the policy. Accidents resulting from dangerous races. AIDS, HIV, infertility. Epidemics, earthquakes, natural catastrophes. War risks, riots, civil war, civil disobedience, radiation, nuclear explosions.
